Privacy Notice
This notice describes the practices of the Nanaimo Division of Family Practice regarding the collection, use and disclosure of personal information. Safeguarding personal information is a fundamental priority for the Division. The Division complies with the Personal Information Protection Act (PIPA) and deems personal information as any information about a person that allows that person to be identified.
Information Collected
​The personal information you provide us directly includes:

profile/contact information (i.e. date of birth, postal address, province, county of residence, e-mail address, telephone number and emergency contact details.

Bank account details and SIN for completing member remunerations.

Privacy Principles
The Nanaimo Division privacy principles form the basis of the Nanaimo Divisions full privacy policy. These principles and overall policy are reviewed annually to ensure relevancy and alignment with provincial and federal legislation.

1. Accountability
The Nanaimo Division is responsible for personal information in its possession and has a designated Privacy Officer who is accountable for the organizations compliance with established privacy principles.
2. Identifying Purposes
The Nanaimo Division will ensure the collection of personal information is necessary to fulfill the purpose identified. When using personal information for a new purpose not previously identified, the Privacy Officer will inform the individual of the new purpose and obtain consent prior to its use.
3. Consent for the Collection, Use, or Disclosure
The Nanaimo Division obtains consent for the collection, use, and disclosure of personal information before or at the time of collection. The purpose of the collection, use or disclosure will be clearly and transparently communicated.
4. Limiting Collection
The Nanaimo Division collects only the personal information necessary to fulfill the purposes identified for the information. Information collection practices are fair, lawful, and respectful of the individual.
5. Limiting Use, Disclosure and Retention
The Nanaimo Division uses and discloses information only for the purposes identified before or at the time of collection. New uses or disclosures are permissible only with the consent of the individual or as required/permitted by law.

The Nanaimo Division ensures that personal information being collected is accurate, complete, and up to date for the purposes for which the information is collected, used, or disclosed. Updates are completed when necessary to fulfill the purpose(s) for which the information was collected.
