What is a Primary Care Network?
Primary care is the everyday health care you receive in your community - usually outside of a hospital. It's your first stop when you need medical help, advice or support to stay on top of your health and can often prevent the need for hospital visits.
PCN Workstreams
Within a team-based care model of care, multiple health care providers from different professional backgrounds work together and with patients/clients, families, caregivers and communities to deliver comprehensive health services across care settings. Effective teamwork is a critical enabler of safe, high quality care and allows health care teams to work to their strengths to provide the best care for their patients.

The Patient Attachment Initative (PAI) is a referral-based program that helps unattached patients with higher or more complex needs living in Nanaimo to find a primary care provider. The PAI works with referral partners within acute, outpatient, inpatients and speciality services.
